Tesla's driver assist Autopilot system was not engaged at the time of a fatal Model S crash that killed all three occupants of the electric vehicle, investigators found. According to data from the onboard computer, the driver had the control of the vehicle as they crashed into construction equipment.

It could never have been 'caused by Autopilot' anyway; only a reckless, foolhardy, inattentive or impaired driver... something you 'journalists' never seem to be able to understand despite the number of times these stories make the news... but where's the click-bait value in that?.
